Kidney Health Nutrition Support

Chronic kidney disease is highly prevalent in type 2 diabetes, and moderate to severe renal functional impairment (eGFR less than 60 ml/min) occurs in approximately 20-30% of patients.  Renal dysfunction often places patients within increased risk of impaired, typically low/hypoglycemia, glucose levels, making it necessary to alter glucose controlling pharmaceutical interventions often.  It is imperative that patients remain mindful and track closely the type and amount of food and fluid consumed.

SMART Nutrition works with patients to create an individualized patient meal planning template, taking into consideration any medical dietary restrictions (phosphorus, potassium, protein, sodium, water) by your kidney specialist (nephrologist).  We help patients prevent further degradation in kidney health via SMART food choices and resources to plan your daily meals and snacks.
